It begins with a conversation. When you call Serava Homecare, our team will listen carefully to your needs, answer your questions, and explain the types of care available. We’ll talk about your loved one’s health, lifestyle, and schedule so we can recommend the best care options.
Next, a licensed nurse will visit your home for a consultation and full evaluation. During this visit, the nurse will:
Assess the patient’s health, mobility, and safety needs
Review medications, routines, and doctor’s recommendations
Listen to family concerns and care preferences
Develop a personalized care plan tailored to the patient
This step ensures care is not only medically appropriate but also aligned with the family’s goals and expectations.
Based on the nurse’s evaluation, we create a detailed care plan that covers everything from personal care to daily routines. We then carefully match your loved one with a caregiver whose skills and personality are the best fit.
On the first day, your caregiver will arrive on time, ready to provide support. They’ll already be familiar with the care plan and routines. Families are encouraged to share feedback and ask questions. Our team will check in regularly to make sure everything is going smoothly.
At Serava, care doesn’t stop at the first visit. We provide continuous oversight, regular nurse check-ins, and open communication with families. If needs change, we adjust the care plan to make sure support stays consistent and effective.
